Do GLP-1 Drugs Cause Gallstones? What the Evidence Actually Shows

The US label warns that gallstone risk goes up. A meta-analysis of 76 randomized trials agrees — especially at weight-loss doses. A 32,052-patient cohort study published in December 2025 found the opposite direction in one high-risk population. Here is every number, the population it belongs to, and what our own evidence base holds.

Gallbladder problems come up constantly when people talk about GLP-1 medications: in our corpus of 185 distinct community reports, 14.6% mention gallstones or gallbladder trouble — eighth of the 15 effects we track, ahead of headache, dizziness, and hair loss. The clinical evidence behind the worry is real, but it is genuinely more mixed than either a scare headline or a reassurance suggests. This is the fourth piece in our evidence series — after which effects have enough evidence to calibrate a model, the pooled clinical rates, and what patients themselves report — and the first to walk through a single effect end to end.

Three kinds of evidence exist, and they do not all point the same way.

What the regulator-approved label says

The current US prescribing information for Wegovy (semaglutide for weight management) carries a dedicated warning section for acute gallbladder disease. Its first sentence: *"Treatment with WEGOVY is associated with an increased occurrence of cholelithiasis and cholecystitis."* The numbers behind it: in randomized adult weight-reduction trials, cholelithiasis (gallstones) was reported by 1.6% of injection-treated patients versus 0.7% on placebo, and by 2.5% versus 1% for the tablet form.

The label also addresses the obvious confounder head-on: *"Substantial or rapid weight loss can increase the risk of cholelithiasis; however, the incidence of acute gallbladder disease was greater in WEGOVY-treated patients than in placebo-treated patients, even after accounting for the degree of weight loss."* In other words, the manufacturer's own analysis says this is not solely a fast-weight-loss artefact. What a meta-analysis of 76 randomized trials says

A systematic review and meta-analysis published in JAMA Internal Medicine in 2022 (He et al.) pooled 76 randomized trials with 103,371 patients. Randomization to a GLP-1 receptor agonist was associated with an increased risk of gallbladder or biliary disease overall (relative risk 1.37, 95% CI 1.23–1.52), and of cholelithiasis specifically (RR 1.27, 95% CI 1.10–1.47).

Two of its interaction findings matter most for weight-management patients. The association was markedly stronger in weight-loss trials (RR 2.29, 95% CI 1.64–3.18) than in diabetes trials (RR 1.27). And it was concentrated at higher doses (RR 1.56, vs 0.99 at lower doses) and longer duration of use (RR 1.40, vs 0.79 for shorter use). Weight-management dosing is exactly the higher-dose, longer-duration end of that spectrum.

What a large new cohort study found — in the other direction

In December 2025, a retrospective cohort study in the *Journal of Clinical Medicine* (Kazi et al., PMID 41464784) used the TriNetX global health-records network to compare adults who have both type 2 diabetes and inflammatory bowel disease — a population at elevated baseline risk of biliary complications — with and without GLP-1/GIP therapy: 16,026 users propensity-matched to 16,026 non-users. Cholelithiasis occurred in 3.5% of GLP-1/GIP users versus 6.3% of non-users; cholecystitis in 0.8% versus 2.2%. The authors conclude that in this population the drugs "are associated with substantially reduced risks of gallstone-related complications."

Hold the caveats alongside the headline: this is retrospective, outcomes are ICD-10-coded rather than adjudicated, and the population — diabetic patients with bowel disease — is not the typical weight-loss patient. One matched cohort does not overturn a meta-analysis of 76 randomized trials. But it is a 32,052-patient data point that the association is not one-directional in every population, and the authors' call for prospective, agent-specific studies is the right one.

How can both be true?

  • Different populations. The label figure and the meta-analysis describe mostly weight-management and diabetes trial populations against placebo; the 2025 cohort describes a specific high-baseline-risk group against routine care. "Compared to what, in whom" changes the answer.
  • Dose and duration. The meta-analysis found the risk concentrated at higher doses and longer use — so a single pooled number understates the range across regimens.
  • Rapid weight loss itself causes gallstones. Losing weight quickly is an independent, well-established gallstone risk factor regardless of how the weight is lost — though the label's analysis found the drug association persisted even after accounting for it.
  • The honest summary: "do GLP-1s cause gallstones" does not have one number. Randomized-trial evidence says risk goes up, most clearly at weight-loss doses over longer periods — from a low base (1.6% vs 0.7% in the label's trials). At least one large real-world population shows the opposite direction. Anyone quoting a single percentage without naming its population is compressing away the part that matters.

    What our own evidence base holds (as of 26 August 2026)

    Our database currently holds 68 gallstone-tagged data points (61 after excluding the April-2026 seed records we retain for audit trail only), most of them deliberately rate-less — case reports, regulatory warnings, and mechanism papers that discuss the effect without stating a comparable incidence figure, which our extraction rules refuse to force a number onto. The eligible clinical rate base is 4 stated rates from 2 distinct sources, pooling to 3.4% (95% CI 2–7%) with confidence graded "low": a STEP-1 trial record stating 1.6% — the same figure the US label reports across adult weight-reduction trials — and the 2025 TriNetX cohort's 3.5%. Two sources, two genuinely different populations, which is why the interval is wide.

    Worth showing rather than hiding: until 26 August 2026 this base was single-source, and our pooled-rates article published it as 1.6% with confidence "very low." The day the second distinct source landed, the pooled estimate moved to 3.4%. That movement is the system working as designed — every figure we publish carries its source count and confidence grade precisely so a single-source number is never mistaken for a settled one. Gallstones remains below the 10-point bar we require for a fully calibrated statistical model, and the community track's 14.6% is a reporting frequency, not an incidence rate — we never blend the two.
